This short application is designed to help us get to know you a little better before we connect in person. It’s also a chance for you to reflect on why this program might be the right next step.

 

We ask that both the teen and the parent/guardian complete a few short questions. There are no “right” answers; we simply want to hear your honest thoughts, hopes, and perspectives.
